Health Home Care Manager

Unity House of Troy is looking for compassionate individuals to fill Care Manager Openings in our Health Home program.
Responsibilities may include:
Coordinate care between consumer and health care professionals
Assist in obtaining services consumers may require as Medicaid recipients
Work with consumers in assessing goals and implementing a care plan to better manage chronic behavioral health, medical and substance abuse conditions Work collaboratively with providers another community service providers to promote primary and preventative care
Assist in reducing or eliminating the need for a hospital/ ER visits
Requirements: A Bachelor’s Degree in Human Services or similar field and two years experience working with individuals living with mental illness, substance abuse or other disabilities, or Bachelor's Degree in any field with five years experience in human services. Proficient with Microsoft Office and other web-based programs. Ability to maintain documentation within an Electronic Records system and complete documentation requirements in a timely manner. Must have reliable transportation, and a valid New York State Driver License and acceptable driving record that meets the requirements of our liability insurance. It is helpful if you have prior experience with Medicaid delivery system.
